Intake & Placement Coordinator

A prominent Health Clinic is seeking a dedicated Spanish Intake Coordinator to join their compassionate team. This crucial role involves facilitating the initial intake process for Spanish-speaking patients, ensuring they receive seamless access to healthcare services. The ideal candidate will be fluent in Spanish and English, possess excellent interpersonal skills, and have a deep commitment to patient care and service excellence.

Serve as the primary point of contact for Spanish-speaking patients seeking services at the clinic.
Conduct initial screenings and assessments to gather patient information and medical history accurately.
Schedule appointments, coordinate referrals, and assist patients with completing necessary forms and documentation.
Provide professional interpretation services during medical appointments, ensuring clear and accurate communication between patients and healthcare providers.
Translate medical documents, instructions, and educational materials from English to Spanish and vice versa as needed..
Assist in cultural competency training for clinic staff to enhance communication and understanding with Spanish-speaking patients.
Advocate for patients' needs and preferences within the healthcare system, ensuring they receive respectful and equitable treatment.
Educate patients about available services, health resources, and financial assistance programs.
Follow up with patients to address any concerns, questions, or additional needs they may have after their initial intake.
Maintain accurate and confidential patient records in compliance with clinic policies and regulatory requirements.
Assist with administrative tasks such as answering phone calls, scheduling appointments, and managing patient inquiries.
Work collaboratively with healthcare providers, administrative staff, and community partners to optimize patient care delivery and coordination.
Participate in team meetings, training sessions, and quality improvement initiatives to enhance clinic operations and patient outcomes.
Fluent in both Spanish and English (written and verbal).
High school diploma or equivalent
Proven experience in a customer service or healthcare-related role.
Strong interpersonal skills and cultural sensitivity.
Ability to multitask, prioritize responsibilities, and work effectively in a fast-paced environment.
Proficiency in basic computer applications and electronic medical records (EMR) systems.
