Resident Care Coordinator

A healthcare facility is seeking a Resident Care Coordinator (MDS). This position entails coordination resident care plans, reviewing nursing documentation, and presenting individualized care plan for each resident.

Responsibilities:

  • Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement.
  • Determine potential Resource Utilization Groups (RUCs) and expense associated with a potential admission.
  • Complete and assure the accuracy of the MDS process for all residents.
  • Maintain current working knowledge of Medicare/Medicaid criteria, serving as a resource for nursing staff and communicate changes in regulations.
  • Monitor Case Mix Index (CMI) scores, looking for potential risks, anti/or changes that may affect Medicaid reimbursement.
  • Facilitate problem-solving for complicated admissions.
  • Monitor assessment schedules and nursing documentation to ensure accuracy and timely submission.
  • Ensure care plans are accurate and complete.
  • Communicate with Administrative staff regularly.
  • Proactively solve quality pitfalls by recognizing the clues reflected in your Quality.
  • Measure and Five Star reports.
  • Uncover unique solutions that honor resident preference by synthesizing MDS, CAA, and care plan data into individualized interventions.

Requirements:

  • 2-years’ experience in MDS role or related experience.
  • Current and valid CT nursing license (LPN or RN).
  • Experience in a clinical and healthcare setting.
  • Deep medical terminology knowledge.
  • Exceptional mathematical skills and ability to apply then to tasks such as analysis of variance and sampling theory.
  • Excellent leadership skills.
  • Demonstrable knowledge of state and federal regulations.
  • Superb decision-making skills.
  • Proficiency in MDS 3.0.

    Location: New Haven, Connecticut
    Salary: $85K
